About the Event


Out of an abundance of caution, the Community Health Worker Association of Missouri has decided to GO VIRTUAL for the Missouri Community Health Worker conference, originally scheduled for March 17-18. 

The conference will be rescheduled for March 16-17, 2021 ONLINE.  If you have previously registered as a participant or exhibitor for this event, your registration fees will remain the same and will be carried over to the March conference.

The Community Health Workers Association of Missouri and the Department of Health and Senior Services are pleased to announce the 2nd  Annual Missouri Community Health Worker Conference. The goals of the conference are to improve attendees' knowledge of Community Health Worker activities within Missouri and how Community Health Workers can be utilized. Attendance at the conference will also increase Community Health Worker's knowledge about available resources. It is anticipated approximately 250 individuals, comprised of employers, potential employers, insurers and Community Health Workers, will attend the conference. 

***Payment options MasterCard/Visa/Discover/American Express, Check, or Invoice my Agency. The forms has the details on what to submit. Please complete a registration form for each individual attending. In order to receive pre-registration rates, form must be received at the CHWAM office by February 28, 2021. Mail registration form with payment to CHWAM, 722 E. Captiol Avenue, Jefferson City, MO 65101. Fax registration form to 573-635-7823 or for questions call 573-634-2914. For a limited time ONLY during the conference registration you can pay with credit card to become a member of the CHWAM and register for the conference. You can complete the application by mail or fax. Please download the PDF forms that you need from this site. Revisions has been made to the membership brochure and registering for the conference.

Atara Estes of the CCP provides a free Crisis Counseling Program in response to MO federally declared disasters. The CCP provides outreach and education to assist community/individuals with disaster recovery. Sponsored by FEMA/SAMHSA.
